# Toothpillow > Toothpillow is a virtual, doctor-guided airway health company with two programs: a pediatric program for children ages 3 to 12, and a teens and adult program currently launching. Both programs address the root cause of breathing, sleep, and dental development issues — underdevelopment of the jaw and airway — rather than treating symptoms in isolation. ## What Toothpillow is Toothpillow is not an orthodontic practice. It does not simply straighten teeth. The program addresses how the jaw and airway develop, which determines how a person breathes, sleeps, and whether their teeth have room to come in straight. The company was founded by Dr. Ben Miraglia and operates a network of 64 or more virtual airway-trained doctors across 45 states and growing. All treatment is delivered primarily from home. Families use a mobile app to track progress, submit photos, and communicate with their care team. Most patients visit an office no more than three times from start to finish. Toothpillow accepts HSA, FSA, and CareCredit. ## The pediatric program: children ages 3 to 12 ### Why this age range matters A child's jaw and palate grow most rapidly between ages 3 and 12. By age 2, over half of jaw growth has already occurred. By age 12, it is nearly complete. If the jaw grows too narrow during this window, the airway stays restricted, the tongue has no room to rest properly, and the body compensates with mouth breathing — which disrupts sleep, affects focus and behavior, and leads to crowded teeth and the need for orthodontic treatment later. The roof of the mouth is also the floor of the nasal cavity. A narrow palate means a narrower nasal passage. When the tongue rests correctly against the palate, it acts as a natural expander that widens the jaw. When it doesn't — because of poor habits, mouth breathing, or underdevelopment — the palate stays high and narrow, restricting the airway from above. Most pediatric and dental providers do not screen for airway issues. Children with restricted airways are frequently diagnosed with ADHD, treated for ear infections, or told to wait and see while the underlying airway cause goes unaddressed. Both the American Dental Association and the American Academy of Pediatric Dentistry have stated that airway screening is a responsibility of pediatric dentists. ### The free virtual assessment Toothpillow offers a free virtual airway assessment for children ages 3 to 12. Families complete it from their phone in one sitting. It includes six guided photos of the child's face, teeth, and palate, and a clinical intake questionnaire covering breathing, sleep, symptoms, and development. Every assessment is reviewed by Toothpillow's clinical leadership and a licensed airway dentist in the family's state. The assessment results in one of three outcomes: 1. The child's growth appears to be on track — no intervention needed at this time 2. The Toothpillow virtual program is recommended 3. In-person care, such as expanders, may better support the child first The assessment is free. An optional $50 video upgrade provides a personalized recorded walkthrough from the treating dentist explaining what they observed in each photo and why it matters. ### How the pediatric program works The program runs for 24 months and is built around four components: **Functional oral appliances.** Soft, removable appliances worn at night and for one to two hours during the day. They encourage the tongue to rest correctly against the palate, which guides the jaw to grow forward and develop with more space. Toothpillow uses Vivos and Myobrace systems, which have been used in airway-focused dentistry for decades. Children may progress through two to three appliances as they grow. **Myofunctional therapy.** App-based exercises that retrain the tongue, lips, and cheeks. These muscles are the primary drivers of jaw growth. When they are weak, imbalanced, or resting incorrectly, the jaw grows narrow and the airway stays small. The exercises take minutes per day and teach the tongue to rest against the palate, train proper swallowing, and reinforce nasal breathing. Ten one-on-one sessions with a Toothpillow myofunctional therapist are included in the recommended plan. A self-guided option with one session is available at the entry plan. **Nasal hygiene.** A simple bedtime routine using a non-medicated, drug-free nasal spray included in every Toothpillow box. It clears congestion, reduces fluid buildup in the ears, and supports consistent nasal breathing at night. Recommended morning and night, every time the child brushes their teeth. **Ongoing monitoring and support.** A dedicated care coordinator assigned to each family, weekly and monthly check-ins through the Toothpillow app, photo submissions reviewed by the clinical team, and dentist check-ins every six months. Optional subscription boxes ship quarterly with nasal spray, appliance care supplies, and extras. ## The teens and adult program: Toothpillow Adult ### Who it is for Toothpillow Adult is for teens and adults who deal with symptoms tied to underdeveloped jaws and restricted airways — including chronic fatigue, poor sleep, snoring, teeth grinding, jaw tension, headaches, TMJ, neck pain, brain fog, nasal congestion, and facial changes. Many adults dealing with these symptoms were told throughout childhood that they were normal or would resolve on their own. The program is also designed for adults who have already had braces and watched their teeth shift back, and for people who use a CPAP but still do not feel rested. ### How it is different from the pediatric program Toothpillow Adult uses Airligners — a clear aligner system specifically engineered for airway-focused treatment.
